First Visit
We want your first visit to be comfortable and easy. Please arrive 15 minutes early with your insurance card, or download and complete your forms in advance.
For adults, we reserve 60 minutes for a complete exam, which includes the following:
- A tour of our state-of-the-art office
- New patient exam
- Charting, including perio
- A review of your health history
- X-rays when necessary
- Imaging
- Hygiene evaluation
- A treatment and full cleaning
Your dentist here at Avita Dental Associates will guide you through their findings and work closely with you to create a dental plan designed to meet your own oral health goals.
For children, as long as the child is comfortable, the appointment includes time for the dentist and the child to get to know one another, complete an exam, and clean your child’s teeth.
Please note: If you need to reschedule your appointment for any reason at all, we are happy to do so with 24 hours’ notice prior to your appointment.

Financial Options
If you do not have an insurance provider, we accept the following:
